2010 Camp Policies

Refund Policy:
$100 administration fee will be retained if you cancel. After July 15, 2010 refunds (less the administration fee) will be prorated only for medical reasons, when accompanied by a letter from a physician.  Refunds may not be mailed until September 2, 2010. If a camper is injured and must leave camp early, the refund will be prorated for room/board only.

Lost Key and Damage Policy:
Athletes and/or their parents/guardians are responsible for any damage to USC property.  This includes, but is not limited to, a lost key fee of $25 and a $75 lock-replacement fee, and a $5 replacement Souvenir Access Card.

Campus Leave Policy:
All athletes are to stay on campus for the entirety of the camp and may not leave without expressed written consent of the Camp Director.  Athletes should be dropped off at campus by a parent or guardian and should not plan to keep a car on campus.

Roommate Policy:
USC will make every effort to place an athlete with the roommate that he requests in his registration, however USC reserves the right to make changes as necessary.  Athletes and their guardians can request a change up to one week prior to the start of the camp.

Agreement and Release of Liability:

The athlete (“Athlete”) should complete this Agreement and Release of Liability (“Agreement”) if the Athlete is age 18 or older. Athlete’s parent or legal guardian should complete and sign this Agreement if Athlete is under age 18.

Location: University of Southern California, Los Angeles, CA

I, the undersigned (or parent/guardian, if Athlete is under 18 years old), understand that this is a legally-binding agreement and release of liability of the University of Southern California (“USC”), USC Men’s Lacrosse (“USC Lacrosse”), the Director(s), and Staff of the Camp.

I/Athlete requests permission to participate in the event(s) identified above. In consideration of permission being granted to me/Athlete to participate in the event activities, I/Athlete agree as follow:

1. Voluntary Activity: I/Athlete understand and agree that my/Athlete’s participation in the event activities is purely voluntary and is not required by USC Lacrosse.

2. Release of Liability: I, on behalf of myself/Athlete, my/Athlete’s heirs, personal representatives, guardians, successors, and assignees, hereby release USC Lacrosse and USC  and their administrators, faculty, trustees, officers, directors, employees, volunteers, coaches, athletic trainers, physicians, and agents (all of whom are referred to as “Releases”) from, and agree not to sue Releases, for any claims, loss, liability, demands, causes of action, costs expenses (including but not limited to attorneys’ fees), damages or suits of any type, whether in law or in equity, that I/Athlete may have arising from, or relating in any way (directly or indirectly) to my/Athlete’s participation in the event activities, including without limitation any physical, emotional or mental injury or property damage that I/Athlete may suffer as a result of my/Athlete’s participation in the event activities, to the maximum extent permitted by law.

3. Acknowledgement of Risk: I recognize and appreciate the dangers, hazards, and risks associated with participation in the event activities. I understand that the dangers, hazards, and risks of the event activities could include serious or even fatal injuries and property damage. I acknowledge that I have fully considered the dangers, hazards, and risks associated with my/Athlete’s participation in the event activities, and voluntarily assume those dangers, hazards, and risks. I give my consent and approval for my/Athlete’s participation in the event activities.

4. Emergency Medical Treatment: I understand and agree that USC Lacrosse and USC may not have medical personnel available at the location of the event activities. I hereby grant USC Lacrosse and USC permission to authorize emergency medical treatment, if necessary, and to transport me/Athlete to an appropriate facility to receive emergency medical treatment, and that such action shall be subject to the terms of this Agreement. I understand and agree that USC Lacrosse and USC assume no responsibility for any injury or damages which might arise out of, or in connection with, such authorized emergency medical treatment.

5. Fitness to Participate: I hereby represent that I am/Athlete is physically and mentally able to participate in the event activities and that I have/Athlete has no health problems or physical or mental conditions that would present a risk to me/Athlete or to others.

6. Drug and Alcohol Policy: I hereby represent that I am/Athlete understands that I/athlete will not participate if I/Athlete is under the influence of illicit drugs and/or alcohol and further more that if found to be in possession of or under the influence of illicit drugs and/or alcohol during an event will be removed from the event immediately at my/Athlete’s expense and ineligible to return for the remainder of the event without compensation of any kind.

7. Insurance: I represent that I am/Athlete is covered by a comprehensive medical plan (health insurance) necessary to provide and pay for any and all medical costs (including but not limited to transportation costs associated with obtaining medical care) and/or I will assume all responsibility for medical costs incurred as a result of illness and/or as a result of my/Athlete’s participation in the event activities. I agree to pay for any costs related to my/Athlete’s medical treatment that are not covered by insurance or if I/Athlete has no medical insurance.

8. Photographs: I consent to the use by USC Lacrosse and USC of any type of images and recordings (including but not limited to photographs and videos) of me/Athlete for publicity, promotion, advertising, or other legitimate purposes.

I acknowledge that I have carefully read this Agreement and fully understand its contents. I acknowledge that I am voluntarily executing this Agreement of my own free will after having the opportunity to consult with legal counsel of my own choosing. I understand that this Agreement means I am/Athlete is giving up, among other things, rights to sue USC Lacrosse and USC and Releases for injuries, damages, or losses I/Athlete may incur. I also understand that this release binds me/Athlete, as well as my/Athlete’s heirs, executors, administrators, and assigns. I further acknowledge and understand that this Agreement will absolve and release USC Lacrosse and USC and Releases from any liability in connection with any injury or harm suffered as a result of my/Athlete’s participation in the event activities. I acknowledge that I have been made aware of any and all risks of participation in the event activities.

I have read and understand that this Agreement is a release of legal rights and claims, and am fully competent to sign this Agreement; and that I execute this Agreement for full, adequate, and complete consideration fully intending for myself, and for the Athlete’s family, estate, heirs, administrators, personal representatives, or assigns to be bound by same.

I further state that I am the Athlete’s parent/guardian, and am fully competent to sign this Agreement, and that I execute this Agreement for full, adequate, and complete consideration fully intending for myself, for the Athlete, and for the Athlete’s family, estate, heirs, administrators, personal representatives, or assignees to be bound by the same.
